Description
OPIS PRZEDMIOTU ZAMÓWIENIA: 1. Przedmiotem zamówienia jest: DOSTAWA LEKÓW ONKOLOGICZNYCH dla Wojewódzkiego Szpitala Specjalistycznego im. Najświętszej Maryi Panny w Częstochowie. Przedmiot zamówienia obejmuje 15 części. 2. Szczegółowy zakres zamówienia został określony w Załączniku Nr 2 do SWZ – formularzu asortymentowo-cenowym. Pozostałe warunki dotyczące realizacji zamówienia zostały określone w projektowanych postanowieniach umowy (PPU), stanowiących Załącznik Nr 6 do SWZ. 3. Wszystkie wymagania określone w dokumentach wskazanych powyżej stanowią wymagania minimalne, a ich spełnienie jest obligatoryjne. Niespełnienie ww. wymagań minimalnych będzie skutkować odrzuceniem oferty jako niezgodnej z warunkami zamówienia na podstawie art. 226 ust. 1 pkt 5 ustawy PZP. 4. Zamawiający dopuszcza składanie ofert częściowych. Pod pojęciem oferty częściowej rozumie się pojedyncze części ustalone w Załączniku Nr 2 do SWZ.
