Description
Αντικείμενο της παρούσας σύμβασης είναι η υλοποίηση ενός ολοκληρωμένου έργου εθνικής εμβέλειας που αφορά τον σχεδιασμό, την ανάπτυξη, την πιλοτική εφαρμογή και την αξιολόγηση ενός σύγχρονου μοντέλου ψηφιακής φροντίδας για άτομα με νευροεκφυλιστικές παθήσεις, με αρχική εστίαση στην άνοια. Το έργο στοχεύει στη δημιουργία ενός ενιαίου και συνεκτικού πλαισίου παροχής υπηρεσιών φροντίδας, το οποίο αξιοποιεί ψηφιακά εργαλεία για την ενίσχυση της ποιότητας, της συνέχειας και της αποτελεσματικότητας της φροντίδας, τόσο σε κλινικό όσο και σε οργανωτικό επίπεδο. Η σύμβαση αφορά ένα πολυδιάστατο έργο, στο οποίο συνδυάζονται οργανωτικές, κλινικές, λειτουργικές και τεχνολογικές παρεμβάσεις. Οι επιμέρους δράσεις δεν είναι αυτοτελείς, αλλά συνθέτουν ένα ενιαίο σύστημα φροντίδας, σχεδιασμένο ώστε να υποστηρίζει την ολοκληρωμένη αντιμετώπιση των αναγκών των ασθενών και των φροντιστών τους, καθώς και τη συστηματική υποστήριξη των επαγγελματιών υγείας.
