Żeromskiego 4/6 i Tarnowskiej 10 wg ilości i asortymentu określonego w załączniku nr 1A, . Szczegółowy opis przedmiotu zamówienia załącznik nr 1A formularz cenowy. Warunki realizacji zamówienia, wymagania dotyczące składania ofert oraz kryteria oceny ofert określono w Specyfikacji Warunków Zamówienia (SWZ) wraz z załącznikami. Warunki realizacji zamówienia oraz zasady płatności zostały określone w projektowanych postanowieniach umowy stanowiących załącznik do SWZ. Zakres i warunki dopuszczalnych zmian umowy zostały wskazane w Rozdziale XXII SWZ.
