Description
1. Przedmiotem zamówienia są dostawy sukcesywne produktów farmaceutycznych dla Oddziału Chemioterapii. 2. Opis przedmiotu zamówienia znajduje się w załączniku nr 2 do SWZ (formularz asortymentowo-cenowy). 3. Zamawiający dopuszcza możliwość składania ofert częściowych (5 – zadań / części zamówienia), zgodnie z podziałem określonym w załączniku nr 2 do SWZ stanowiącym Opis przedmiotu zamówienia: 1) zadanie nr 1 – Bevacizumabum; 2) zadanie nr 2 – Filgrastimum; 3) zadanie nr 3 – Irinotecani hydrochloridum trihydricum; 4) zadanie nr 4 – Paclitaxelum; 5) zadanie nr 5 – Pegfilgrastimum. 4. Zamawiający zobowiązany będzie do wykorzystania minimum 75% wartości brutto udzielonego zamówienia. Wykorzystanie zależne będzie od liczby pacjentów zgłaszających się do Zamawiającego oraz od umów (kontraktów) z Narodowym Funduszem Zdrowia.
