Description
Przedmiotem zamówienia jest zakup i sukcesywna dostawa toksyny botulinowej na okres 12 miesięcy. Oferowane produkty lecznicze muszą być dopuszczone do obrotu i posiadać wskazania zgodne z wymaganiami programów lekowych B.57 i B.28, w zakresie właściwym dla poszczególnych zadań, zgodnie z aktualnie obowiązującymi przepisami i obwieszczeniami Ministra Zdrowia. Wykonawca oświadcza, że oferowane produkty lecznicze posiadają ważne pozwolenie na dopuszczenie do obrotu na terytorium Rzeczypospolitej Polskiej oraz spełniają wymagania określone w Załączniku nr 2 do SWZ. Szczegółowe wymagania dotyczące realizacji zamówienia zostały określone we wzorze umowy oraz formularzu asortymentowo-cenowym stanowiących odpowiednio załącznik nr 5 do SWZ i załącznik nr 2 do SWZ.
