Description
1. Przedmiotem zamówienia jest: sukcesywna dostawa do siedziby Zamawiającego – APTEKA i/lub DZIAŁ FARMACJI - (wraz z transportem i rozładunkiem w lokalu we wskazanym przez pracownika pomieszczeniu) produktów wymieniowych asortymentowo i ilościowo w formularzu asortymentowo-cenowym (zestawy 1 – 15) stanowiącym załączniki nr 2 do SWZ. 2. Szczegółowy opis przedmiotu zamówienia i warunki realizacji określa: a) formularz oferty, który stanowi załącznik nr 1 do SWZ, b) szczegółowy zakres przedmiotu zamówienia określa formularz asortymentowo-cenowy (zestawy 1-15), który stanowi załączniki nr 2 do SWZ, c) projekt umowy, której wzór stanowi załącznik nr 8 do SWZ. 3. Miejscem realizacji dostaw będzie: a. Szpital Pulmonologiczno-Reumatologiczny w Kup, ul. Karola Miarki 14, 46-082 Kup – lokal Apteki Szpitalnej – III p. budynek główny Szpitala w Kup, b.
