Predmetom zákazky je dodávka lieku: burosumab 20mg injekčný roztok a burosumab 30 mg injekčný roztok s účinnou látkou burosumab, známy ako Crysvita (ďalej iba „produkt“), pre detských pacientov s X-viazanou formou hypofosfatemickej rachitídy pri splnení všet-kých nižšie vymenovaných kritérií: 1) s dokumentovanou vysokou sérovou koncentráciou FGF-23 2) s geneticky potvrdenou mutáciou v PHEX géne 3) perzistencii nízkej hypofosfatémie napriek dlhodobej substitúcii maximálnymi tolerovanými dávkami fosfátového roztoku a aktívnej formy vitamínu D 4) pri progresii kostných deformít napriek podávaniu maximálnych tolerovaných dávok štandardnej lieč-by (fosfáty a vitamín D).
