Description
Inkoopdoel van de opdracht is vanaf 1 januari 2028 één aanbieder contracteren voor onafhankelijke cliëntondersteuning (Wmo 2015) die inwoners gratis en onbevooroordeeld helpt bij vragen in het sociaal domein. De gevraagde resultaten zijn onder meer: - daadwerkelijke onafhankelijkheid en objectieve informatie; - laagdrempelig en toegankelijk zijn voor alle inwoners; - versterking van zelfredzaamheid en participatie; - passende toegang tot voorzieningen; - bevordering van inclusie en gelijke kansen; - levering van kwaliteitsvolle dienstverlening; - en goede samenwerking met gemeentelijke partners. Daarnaast wordt continu leren en verbeteren gestimuleerd door evaluatie, transparante rapportages en afstemming met relevante partijen. Het betreft een opdracht voor 4 jaar, met twee maal de optie tot verlenging.
