Description
Przedmiotem zamówienia jest zakup wraz z dostawą na zlecenie Zamawiającego wyrobów medycznych pn. „DOSTAWA WYROBÓW MEDYCZNYCH CZ. IV – PRODUKTY DO TERAPII ODDECHOWEJ I ANESTEZJOLOGII”, szczegółowo opisanych w załączniku nr 2 do SWZ – Formularz asortymentowo-cenowy oraz w załączniku nr 6 do SWZ – Projektowane postanowienia umowne. Przedmiot zamówienia został podzielony na 17 części, zwanych dalej „Pakietami”. Zamawiający dopuszcza składanie ofert częściowych. W ramach zamówienia wyodrębniono następujące Pakiety: Pakiet nr 1 – worek do ssaka próżniowego. Pakiet nr 2 – rurki tracheostomijne, intubacyjne – specjalistyczne. Pakiet nr 3 – obwody oddechowe dla dorosłych. Pakiet nr 4 – obwody oddechowe dla dzieci. Pakiet nr 5 – produkty do podawania tlenu – dreny, wąsy. Pakiet nr 6 – maski krtaniowe. Pakiet nr 7 – maski anestetyczne, tlenowe, filtry, wymienniki. Pakiet nr 8 – kapnolinia. Pakiet nr 9 – bronchoskop jednorazowy w 3 rozmiarach. Pakiet nr 10 – bronchoskop jednorazowy w 2…
