1. Przedmiotem zamówienia jest dostawa odczynników, testów i podłoży do diagnostyki infekcji wirusem HPV wraz z dzierżawą odpowiednich urządzeń zwanych w dalszej części SWZ: „towarem” na potrzeby Samodzielnego Publicznego Zakładu Opieki Zdrowotnej Wojewódzkiego Szpitala Zespolonego w Białymstoku, w ilości, asortymencie oraz parametrach określonych w Załączniku nr 2 do SWZ – Formularz Szczegółowy Oferty. 2. Opis przedmiotu zamówienia oraz warunki realizacji zamówienia zawarte są w załącznikach: a) Formularz Szczegółowy Oferty (opis przedmiotu zamówienia) – Załącznik nr 2 do SWZ; b) Projekt umowy (warunki realizacji zamówienia) Załącznik nr 7 do SWZ.
