1. Przedmiotem zamówienia jest: Dostawa odczynników laboratoryjnych i testów diagnostycznych na potrzeby diagnostyki histo-patologicznej, mikrobiologicznej i HLA na potrzeby USK-2 PUM w Szczecinie 2. Ilekroć w dalszej części SWZ użyty będę zwrot: a) „wyroby” – należy pod tym pojęciem rozumieć wyroby i materiały zużywalne wskazane w załączniku nr 2 (FCJ) do SWZ; b) „sprzęt” - należy pod tym pojęciem rozumieć dzierżawiony aparat do odczytu wyników z podłoża do badania czystości mikrobiologicznej powierzchni -dotyczy zadania nr 7. 3. Opis przedmiotu zamówienia oraz szacowane ilości dotyczące wyrobów zostały zawarte w formularzu cen jednostkowych (zał. 2 do SWZ, dalej również jako FCJ oraz zał 2a OPZ dla zadania nr 5). Formularz cen jednostkowych zał 2 do SWZ winien bezwzględnie być złożony wraz z formularzem oferty (zał. 1 do SWZ, zwany dalej również jako „FO”) 4.
