1. Przedmiotem zamówienia jest realizacja zadania pn.: „Dostawa urządzeń, siatek, staplerów oraz innego wyposażenia medycznego na potrzeby Kliniki Chirurgii Ogólnej i Onkologicznej - Bloku Operacyjnego Centralnego Szpitala Klinicznego Uniwersytetu Medycznego w Łodzi” – 10 pakietów, zgodnie z asortymentem i ilościami określonymi w Formularzu asortymentowo - cenowym stanowiącym Załącznik Nr 2 do SWZ. Przedmiot zamówienia został szczegółowo opisany w Załączniku Nr 2 do SWZ – Formularzu asortymentowo - cenowym. UŻYCZENIE: Dot. Pakietu nr 2 - Zamawiający wymaga użyczenia na czas trwania umowy sprzętu opisanego w załączniku nr 2 do SWZ. Zamawiający dopuszcza możliwość składania ofert częściowych na poszczególne pakiety. W ramach pakietu Zamawiający wymaga złożenia oferty pełnej.
