Description
Przedmiotem zamówienia jest dostawa staplerów chirurgicznych oraz ładunków do staplerów dla potrzeb CPiT w Bystrej, szczegółowy opis przedmiotu zamówienia w asortymencie i ilościach został określonych w formularzu asortymentowo-cenowym - Zał. nr 2 do SWZ. Zamówienie składa się z 3 Pakietów. Zamawiający dopuszcza składanie ofert częściowych na poszczególne Pakiety. Każdemu z wykonawców przysługuje możliwość złożenia oferty na dowolną ilość wybranych przez siebie Pakietów, z zastrzeżeniem, że oferta musi zawierać wycenę całego asortymentu w ramach Pakietów, na które Wykonawca składa ofertę. Wymaga się daty ważności oferowanego asortymentu min. 6 miesięcy od momentu dostarczenia zamówienia do Zamawiającego. Zamawiający wymaga, aby wszystkie oferowane produkty były wprowadzone do obrotu i stosowania na terytorium RP, sklasyfikowane według obowiązujących przepisów: Rozporządzenia Parlamentu Europejskiego i Rady (UE) 2017/745 z dnia 5.04.2017 r. w sprawie wyrobów medycznych, Rozporządzenia…
