Description
1. Przedmiotem zamówienia jest: Akcelerator do tomoterapii wraz z modernizacją bunkra. Przedmiot zamówienia obejmuje: 1) zakup przez Zamawiającego oraz sprzedaż przez Wykonawcę fabrycznie nowego akceleratora do tomoterapii (zwanego dalej "sprzętem") 2) wykonanie dokumentacji projektowej oraz robót budowlanych i instalacyjnych związanych z instalacją sprzętu oraz adaptacją pomieszczenia w Zakładzie Radioterapii I Zamawiającego, w celu dostosowania do wymagań funkcjonalno-technicznych, wykonanie wszelkich robót w ramach prac adaptacyjnych i remontowych objętych przedmiotem zamówienia 3) demontaż, wyniesienie, transport oraz utylizację urządzenia, znajdującego się na wyposażeniu Zamawiającego, zgodnie z obowiązującymi przepisami prawa, wraz z przekazaniem Zamawiającemu dokumentów potwierdzających prawidłowe zagospodarowanie odpadów 4) dostarczenie, instalację, uruchomienie i oddanie do eksploatacji sprzętu; 5) przeprowadzenie szkolenia dla minimum 6 pracowników (zgodnie z SWZ)…
