Description
1. Przedmiotem zamówienia jest zakup i sukcesywna dostawa nici chirurgicznych i kleju tkankowego, wyszczególnionych w Częściach nr 1–44, dla Uniwersyteckiego Szpitala Klinicznego im. Fryderyka Chopina w Rzeszowie, w tym: Części nr 1–33 oraz 35 - 44 – nici chirurgiczne; Część nr 34 – klej tkankowy. 2. Szczegółowy opis przedmiotu zamówienia oraz wymagania Zamawiającego określono w Formularzach cenowo–ofertowych stanowiących Załącznik nr 1 do SWZ dla poszczególnych Części, będących integralną częścią SWZ. 3. Zamawiający wymaga, aby oferowane przedmioty zamówienia: 3.1. spełniały wymagania zawarte w Formularzach cenowo – ofertowych. 3.2. w Częściach nr 1–44 były wyrobami medycznymi zgodnymi z ustawą z dnia 7 kwietnia 2022 r. o wyrobach medycznych (Dz. U. z 2024 r. poz. 1620, z późn. zm.) oraz z mającymi zastosowanie przepisami prawa Unii Europejskiej, w szczególności rozporządzeniem Parlamentu Europejskiego i Rady (UE) 2017/745 (MDR), w zakresie właściwym dla danego wyrobu, w tym…
