Description
I. Przedmiotem zamówienia jest dostawa elektrod EEG oraz innego drobnego sprzętu i materiałów medycznych. Szczegółowy opis oraz sposób realizacji zamówienia zawiera tabela asortymentowo-cenowa zał. nr 2 do SWZ i wzór umowy zał. nr 5 do SWZ. II. Zamawiający podzielił przedmiot zamówienia na 8 części. Zamawiający dopuszcza możliwość składania ofert częściowych. Ofertę można składać w odniesieniu do wszystkich części zamówienia. III. 1. Zgodnie z art. 13 ust. 1 i ust. 2 oraz art. 14 ust. 1 i 2 rozporządzenia Parlamentu Europejskiego i Rady (UE) 2016 /679 z dnia 27 kwietnia 2016 r. w sprawie ochrony osób fizycznych w związku z przetwarzaniem danych osobowych i w sprawie swobodnego przepływu takich danych oraz uchylenia dyrektywy 95/46/WE (ogólne rozporządzenie o ochronie danych) (Dz. Urz. UE. L. z 2016 r.
