Description
1. Przedmiotem zamówienia jest sukcesywna dostawa sprzętu medycznego dla Pracowni Hemodynamiki oraz Kardiologii Inwazyjnej UCK WUM w podziale na 5 Części: Część nr 1 Filtry żylne oraz pętle do usuwania filtra z żyły głównej dolnej Część nr 2 Introducer sterowalny jednokierunkowy Część nr 3 Koszulki naczyniowe do dostępu przez tętnicę udową Część nr 4 Cewniki diagnostyczne do koronografii Część nr 5 Systemy do trombolizy oraz trombektomii 2. Szczegółowy opis przedmiotu zamówienia stanowi Załącznik nr 2 do SWZ - Formularz asortymentowo-cenowy. Wykonawca zobowiązuje się do utworzenia w siedzibie Zamawiającego na własny koszt i ryzyko magazynu depozytowego (dotyczy Części nr 1). Miejscem utworzenia depozytu jest: a) Pracownia Hemodynamiki SKDJ UCW WUM ul. Lindleya 4, 02-005 Warszawa b) Oddział Kardiologii Inwazyjnej CSK UCK WUM ul. Banacha 1, 02-097 Warszawa
