1. Przedmiotem zamówienia jest świadczenie usługi serwisu i nadzoru autorskiego medycznego systemu informatycznego RIS/PACS Alteris przez okres 36 miesięcy. 2. Zamawiający nie dopuszcza składania ofert częściowych. 3. Szczegółowy opis przedmiotu zamówienia zawiera załącznik nr 3 do swz. 4. Wykonawca jest zobowiązany do zrealizowania zamówienia na zasadach i warunkach określonych w projekcie umowy stanowiącym załącznik nr 5 do swz. 5. Zamawiający informuje, że w ramach realizacji umowy o udzielenie zamówienia publicznego przewiduje możliwość skorzystania z prawa opcji – szczegółowe postanowienia dotyczące realizacji opcji zawiera projekt umowy stanowiący załącznik nr 5 do swz.
