1. Przedmiotem zamówienia niniejszego postępowania jest usługa transportu sanitarnego pacjentów oraz przewozu lekarza do pobierania materiałów biologicznych, przewóz próbek do badania i / lub przewozu zespołu transplantacyjnego wraz ze sprzętem w miejsce pobrania narządów do przeszczepu dla Uniwersyteckiego Szpitala Klinicznego Nr 1 im. N. Barlickiego w Łodzi (zwana dalej usługą), zgodnie z rodzajem i ilościami określonymi w „Formularzu asortymentowo-cenowym” stanowiącym załącznik nr 2 do SWZ i zgodnie z Formularzem Oferty, stanowiącym załącznik nr 1 do SWZ oraz Szczegółowym opisem przedmiotu zamówienia stanowiącym załącznik nr 1a i 2 do SWZ.
